Please call at least 6-8 weeks in advance for all camp, school and sports physicals. We will do our best to try to accommodate your requested physician and dates. For school entrance exams (Kindergarten & 6th grade), please make them as far in advance as possible. School entrance exams can be done as much as one year prior to entry into school. Don't wait until the last minute. Thank you.

Please bring any required school, sports or camp forms to your checkup appointment so that the form can be completed by our staff. The required High School Athletics form can be found on our website under Office Forms.
