Our records regarding the patient’s health will be kept by our office. We will release copies of medical records only if requested in writing by the parent or legal custodian. If transferring records to another physician, you must complete an Authorization for Release form and send or fax to our office. Please allow 2-3 weeks for us to copy your chart. Kentucky State Law requires that one copy of your records be provided free of charge. If further copies are requested, a fee of $1 per page will be charged.
